Chronic Pain Shouldn’t Be Overlooked in Rising Temperature Concerns

When a heat wave occurs, public health advice generally emphasizes hydration, monitoring for heat stroke, and checking on people with heart or lung conditions. While this advice is life-saving, it often overlooks chronic pain, which significantly affects millions of older Americans. A recent study by Kai Zhang, a public health professor, and myself tracked over 35,000 adults aged 50 and above across two decades, aligning their reports of chronic pain with the temperature history in their areas.

Impact of Long-Term Exposure to Extreme Temperatures

The study aimed to determine whether prolonged exposure to extreme heat or cold affects the degree to which pain interferes with people’s lives. The findings indicate a need for chronic pain to be included when preparing communities for extreme weather conditions. Notably, rural residents appear to be the most vulnerable.

Understanding High-Impact Pain

High-impact chronic pain refers to persistent pain lasting at least three months, limiting everyday activities like work, socializing, and self-care. It’s not occasional discomfort but pain that influences major life decisions. This matters significantly for those over 50, as pain that hinders standing, carrying groceries, or sleeping uninterrupted often leads to losing the ability to live independently.

Temperature’s Role in Pain Aggravation

Different temperatures affect the body variously, leading to pain. Cold weather seems to heighten pain sensitivity and stiffen joints, a common concern for those with arthritis during winter. On the other hand, heat contributes to pain through dehydration, interrupted sleep, and reduced daily activity. For instance, hot days may discourage walking, resulting in stiffer joints and weaker muscles, exacerbating pain.

Findings and Regional Differences

The study measured when each neighborhood experienced temperatures considered extreme for that locale. Maine’s threshold for extreme heat would be a day exceeding 84 degrees Fahrenheit, while Houston’s falls above 96 degrees. Our analysis assessed communities against their own temperature records from the past decade.

Key observations from the study include:

  • Those facing frequent extreme-cold days reported increased high-impact pain, whereas occasional cold spells seemed more manageable.
  • Urban residents did not show a strong link between sustained heat and new pain development, but rural residents did.
  • Among poorer rural residents, prolonged heat exposure correlated with a higher likelihood of developing debilitating pain, unlike their wealthier counterparts, who have better access to cooling systems and healthcare.

Lessons Learned for Supporting Older Adults

The findings highlight three crucial lessons for better supporting older adults:

  • Chronic pain should be included in lists of heat-related health risks. State and county plans often focus on cardiac and respiratory concerns, neglecting those whose independence relies on effective pain management.
  • Cooling assistance programs must be tailored to rural needs, ensuring services meet local demands through credible channels, rather than mimicking urban solutions.
  • Medical practices should incorporate housing and climate information into patients’ documented pain history, helping to guide treatment more effectively.

As global temperatures rise, the likelihood of prolonged heat exposure increases. For example, in 2024, Phoenix experienced 39 consecutive nights without temperatures dropping below 90 F. When heat waves occur, especially in rural settings, ensure that inquiries extend beyond hydration, addressing whether chronic pain symptoms have intensified.
