Recently, a trend known as testosterone-maxxing has gained traction on social media, particularly among men seeking to boost their testosterone levels. This approach involves both natural methods and artificial enhancements to increase testosterone, a hormone that plays a crucial role in male health.

Concerns Over Declining Testosterone Levels

A TikTok user, @learningwliam, highlighted research showing a decline in testosterone over the last decade, especially among obese individuals. He suggested that lifestyle changes such as regular exercise, sufficient sleep, a healthy diet, and reducing alcohol consumption might help increase natural testosterone levels.

However, the trend has raised concerns among medical professionals. Some men are pursuing artificial means to increase testosterone, which poses potential health risks if pursued without medical reasons.

Expert Opinion on Artificial Testosterone Use

Dr. Alexander W. Pastuszak, a urologist and associate professor at the University of Utah School of Medicine, warns about the dangers of exogenous testosterone—testosterone introduced from external sources—when not medically indicated. According to Dr. Pastuszak, inappropriate use can cause problems, masking the actual solutions needed for symptoms like fatigue, low libido, or mood issues, which might stem from sleep, weight, or stress issues rather than testosterone.

He noted, “More isn’t better,” emphasizing the need for a physiologic rather than a supraphysiologic level. Supraphysiological doses are those higher than the body’s natural production, a common outcome with over-the-counter supplements.

Social Media’s Role in the Testosterone Trend

Dr. Pastuszak points out that social media influencers and platforms have made testosterone a “proxy for masculinity and vitality.” Influencers have increased concerns about low testosterone, turning it into a marker of social status and perceived masculinity.

He observed a significant shift in how men perceive testosterone, driven by online content, resulting in many seeking treatments for testosterone levels that are within normal ranges and show no symptoms.

Risks of Unregulated Testosterone Supplements

Dr. Pastuszak identifies two major risks associated with over-the-counter testosterone products. First, these products are unregulated, with inaccurate claims, meaning users might not receive safe or effective dosages.

Second, testosterone therapy requires careful monitoring. Risks such as blood thickening, increased estrogen levels, worsened sleep apnea, and suppressed fertility are possible without professional supervision, particularly with unnecessarily high doses.

Dr. Pastuszak stresses that any warranted testosterone therapy should be diagnosed, prescribed, and monitored by qualified professionals to ensure safety and efficacy.
